The Power Couple of Pain Relief: Focused vs. Radial Shockwave Therapy Explained
At Advanced Foot Care of NJ, we love educating our patients and answering these exact questions. As we recently discussed in our clinic’s latest educational video, this technology isn’t science fiction; rather, it is a biologically proven way to jumpstart your body’s natural healing process.
Even better? We don’t just use one type of Shockwave. We utilize both Radial and Focused Shockwave machines. Together, they form what we call the “power couple of pain relief.”
If you are trying to figure out which type of shockwave therapy is best for your specific foot pain, here is a clear breakdown of how these two distinct technologies work and why they are the key to avoiding surgery.
What is Shockwave Therapy (EPAT)?
First, let’s clear up the biggest misconception: there are no electrical shocks involved.
Shockwave Therapy (often referred to as EPAT) uses a handheld wand to deliver high-energy acoustic sound waves into your injured tissue.
- When an injury (like a torn ligament or a fractured bone) stops healing, it gets stuck in a cycle of chronic inflammation.
- The acoustic waves break up stiff scar tissue and create controlled “micro-trauma,” tricking your brain into sending a massive rush of fresh blood, oxygen, and stem cells back to the area to finally finish the repair job.
But not all tissues are the same.
- A ligament near the surface of your skin requires a different type of sound wave than a bone spur buried deep inside your heel.
That is exactly why we use two different machines.
Radial Shockwave: “The Crowd Pleaser”
Radial Shockwave is the most common form of this therapy, and we often refer to it as the “crowd pleaser.”
When the applicator sends the sound waves into your foot, the energy spreads outward in a radial (cone-like) pattern. It covers a larger, wider surface area but does not penetrate incredibly deep. This outward-spreading energy makes it the absolute gold standard for treating superficial, soft-tissue injuries that cover a wider zone of your foot.
Radial Shockwave is perfect for:
- Plantar Fasciitis: If you are dealing with a severe, stubborn case of plantar fasciitis that makes your first steps out of bed agonizing, radial waves effectively target that long band of tissue across the bottom of your foot.
- Superficial Tendonitis: If you feel like your recovery from tendonitis is completely stuck, radial Shockwave is fantastic for mid-substance Achilles and peroneal tendon issues, helping athletes recover from runner’s remorse and get back on the road faster.
Focused Shockwave: “The Precise Deep-Diver”
While Radial Shockwave spreads its energy outward, Focused Shockwave does the exact opposite.
This highly advanced machine takes the acoustic energy and funnels it into a tight, highly precise focal point. Because the energy doesn’t spread out, it can penetrate much, much deeper into the body: up to 12 centimeters beneath the skin.
This allows us to target deep-rooted bone and joint problems without damaging the healthy soft tissue sitting on top of it.
Focused Shockwave is perfect for:
- Bone Spurs: When you have unexplained heel or foot pain that turns out to be a bone spur, focused waves can penetrate deeply enough to target the bony prominence.
- Stress Fractures & Deep Bone Bruises: The focused energy stimulates osteoblasts (the cells responsible for bone formation), rapidly accelerating the healing of hairline fractures.
- Severe Arthritis: If you are suffering from a deep, chronic joint ache, such as the pain on the top of your foot caused by midfoot arthritis, focused Shockwave can reach the joint space to reduce inflammation and improve mobility.
Which One Do You Need?
The beauty of having the “power couple” of shockwave machines in our office is that you never have to settle for a one-size-fits-all treatment.
- In many complex cases, we use both machines during the same treatment protocol to address both the deep bone injury and the surrounding soft tissue inflammation simultaneously!
If you are tired of resting, taking pain relievers, and hoping for a miracle, it is time to trust the science of regenerative medicine.
